Annual reports bring together national learning from serious child safeguarding incidents notified to the Child Safeguarding Practice Review Panel each year, identifying emerging risks, recurring themes and areas where safeguarding practice across England can be strengthened to better support children and families.
Over time, we intend to develop more regular, quarterly reporting to provide earlier insight into emerging themes and pressures, alongside our annual reports.

- Babies under one were most affected, accounting for 34% of rapid reviews and 60% of deaths
- Harm rarely arose from a single issue, with many children living in complex, overlapping circumstances
- Neglect, domestic abuse, parental mental ill‑health and substance use frequently featured together
- Strong multi‑agency working is critical, with reviews highlighting ongoing challenges around information sharing, coordination and risk assessment
