This briefing paper draws on learning from serious child safeguarding incidents involving adolescents who died by suicide. It explores how risk can build over time through cumulative harm and adversity, helping practitioners better understand children’s lived experiences, recognise escalating vulnerability and identify opportunities for earlier intervention.
